Guest Criteria
We have a Welcoming Committee which remains aware of our current abilities and has the power to decide when we can make exceptions to the criteria
Weighs 175 pounds or less
Non-ambulatory / bedbound
Doesn't have complex medical needs (eg. PEG tubes, frequent bowel movements)
No behavioral issues or 1:1
No infectious diseases
In their last days to weeks of life
Enrolled with a partnered hospice
Has a finalized, legal DNR/DNI
The Referral Process
1. First fill out the initial referral questionnaire. This is a fast yes/no questionnaire for screening and prioritizing referrals. It's also a way to track trends and needs in our community, so it's valuable to complete this every time you have a client who would benefit from such a home - even if current Inhora procedures don't enable us to care for them.
2. Next fill out a more detailed referral form however this form is only to be completed after Inhora contacts you because the Initial Referral Form indicates a good fit and there is bed availability.
3. After the Secondary Detailed Form is submitted, the Guest Agreement will be signed and submitted by your patient or their representative AND you’ll send us related medical records.
